The Opportunity:
Appletree Forms Clinic is seeking a detail-oriented and efficient Forms Department Administrator to support our physicians by completing Attending Physician Statements (APS) and medical forms. This is a full-time role ideal for individuals with a background in medicine and a detail-oriented mindset.

Compensation & Career Development:
  • Hourly Wage: $20 per hour.
  • Training Requirements: Successful completion of all required training milestones, exams, and certifications within the first three (3) weeks of employment.
  • Performance Review: Following a three (3)-month probationary period, IMAs are eligible for a formal performance review.

Benefits:
  • Comprehensive benefits package including vision, dental, medical, and paramedical services.
  • Benefits eligibility begins after six (6) months of continuous full-time employment.

What You'll Do:
  • Review patient medical records and encounter notes to accurately identify diagnoses, impairments, treatment plans, and other information needed to complete forms.
  • Complete Attending Physician Statements and other medical questionnaires on behalf of Appletree physicians.
  • Conduct brief patient interviews and finalize required forms within 30 minutes per case.
  • Maintain a daily output of at least 10 completed forms or APSs.
  • Communicate clearly with physicians, patients, and clinic staff to ensure accurate and timely form completion.
  • Collaborate with the clinic team and actively participate in weekly internal and external scrum meetings.

What We're Looking For:
  • Medical degree completed.
  • Successful completion of the MCCQE Part I.
  • Previous residency interviews in Canada or the United States considered an asset.
  • Recent clinical medical experience within the past five (5) years considered an asset.
  • Prior experience in a clinical administrative role is considered an asset.
  • Excellent English communication skills (verbal, written, reading, and comprehension) to effectively interact with patients, families, and healthcare professionals; additional languages are an asset.
  • Permanent eligibility to work in Canada (Canadian Citizen or Permanent Resident), or possession of a valid work permit extending beyond 12 months (no sponsorship available).
  • Minimum typing speed of 40 words per minute for data entry.
  • Strong understanding of medical terminology, clinical procedures, and patient care protocols.
  • Proficiency with electronic medical record (EMR) systems and related clinical software.
  • Basic IT skills and familiarity with computer systems.
